If your department’s conflict of interests (COI) code identifies your position as one that participates in making governmental decisions, you are a designated filer who must file statements of economic interests (SEI).  Below you will find information regarding your filing requirements.

Are you required to file?

Only if your position is designated on your department's conflict of interest code and you assumed office before October 1, 2011.

What are you required to file?

* HACLA, WIB, and IDA Commissioners are not required to file the Form 11.

We are very pleased to introduce eDisclosure, a system that will help automate and streamline the state's Form 700 filing requirements for you.

What does eDisclosure do?
  • Sends you automatic filing reminders;
  • Tells you what filing your are required to complete;
  • Auto-fills your Form 700 cover page;
  • Provides instant feedback about errors; and
  • Stores your information for your next required filing.
Important Reminders:
  • The state still requires each Form 700 to have an original signature; therefore, you will still need to print, sign, and file a paper form with your department’s ethics liaison.
  • The City’s Forms 10 and 11 have not yet been integrated into eDisclosure, so if you are an elected official, department head, board member, and/or commissioner, you will need to download and file a paper form with your Form 700 (Annual only).
  • Late penalties of $10 for the Form 700 and $25 for Forms 10 and 11 will be assessed for each day the forms are filed late.
